Care coordination
- Care coordination assistance to help you navigate the Support at Home program
- Your care coordinator will help you with aspects of your support plan that you may find difficult, such as managing your care workers and support services or organising equipment purchases
Care management
- A dedicated care partner
- Design and ongoing maintenance of your support plan and funding
Funding management
- Assistance managing the financial components of your Support at Home funding, including paying invoices and reimbursements
- Payment management to ensure your services are paid on time with daily payment runs
Compliance support
- Compliance and background checks on all your service providers
Clinical support
- Access to experienced nurses who can support your care plan and help manage any health concerns.
